They could simply use a system similar to what Warframe offers for custom clan emblems. You pay the required platnum (in-game real money currency, which you can get via trading with other players) to upload a size specific .png to their website. Then someone working for the company gets to look over the requested item and decide if the standards are met, ToS isn't broken, etc. If everything is good the symbol gets added to the game (For Warframe, this image gets applied directly over your basic clan emblem) for your use in the following patch.
