When you run out of internal fuel you use up one pip of warp fuel to refill the internal. You can stay out indefinantly, and you will get an alert when you don't have enough warp fuel to fill up the internal tank. There are some ships where the pips used are not visible, like the Hauler and the Adder, just because they have so much fuel.
