Bot account: Link the bot's user page. Use a separate account from the human operator's account. The existing Bots policy applies.

Human operator: Link your user and talk pages.

Public source code and independent review: Link the public source code and evidence of independent review.

Account access: Name everyone with access to the bot account. Do not include credentials.

Task and purpose: Describe the changes and why they are needed.

Scope and exclusions: Identify affected pages or namespaces and what must be left alone.

Method and public examples: Describe the matching rules and link sample diffs or a preview. Do not include credentials or private data.

Proposed trial: State the page or edit limit, pace, duration and checks. Await an explicit human decision before starting the trial.

Monitoring, stop and recovery: Explain how errors will be detected, how to contact the operator, how the bot is stopped and how mistakes can be undone.

Mandatory stop: Confirm that the bot will be stopped if it makes unexpected edits or an administrator requests a pause.

Technical bot flag: State whether it is requested and why. A separate grant by a bureaucrat or operator is needed. Administrators cannot grant it through administrator rights alone.

Trial results: Leave pending until an agreed trial is complete, then add public diffs, checks and any problems for review.
